The Social Security Administration (SSA) administers the Ticket to Work Program. Under this free and voluntary program, eligible beneficiaries between the ages of 18 and 55, who are blind or have a disability and receive Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) benefits are entitled to sign up with an Employment Network (EN) or a State Vocational Rehabilitation (VR) agency of their choice. These approved service providers coordinate and provide appropriate services to help eligible beneficiaries find and maintain employment. 

Quality Insights Renal Network 4 is conducting a Population Health Focused Pilot Quality Improvement Activity (PHFPQ) in 2018 focused on encouraging gainful employment of ESRD patients (Increasing Vocational Rehabilitation Referrals and Utilization).  This project was rolled out in December 2017 for 29 Pennsylvania and 5 Delaware dialysis facilities.  The goal is to increase the referrals to an Employment Network or a Vocational Rehabilitation for eligible patients between the ages of 18-54 and  increase the number of patients enrolled in services.  
 
Participating facilities will be educating patients about the resources available to assist patients in their quest to return to gainful employment.  After the identification of eligible patients, facility social workers will be working with these patients to ensure they are aware of the services available and the process for becoming enrolled.
