Quality Insights Renal Network 4 is conducting a 2020 Population Health Focused Pilot Quality Improvement Activity (PHFPQ) focused on encouraging gainful employment of patients with end stage kidney disease (ESKD). Employment among patients on dialysis is associated with better clinical and quality of life outcomes compared to unemployment.
Goal: Through increased access to services and positive culture change, the Network will support dialysis facilities with:
- Screening at least 95% of patients for eligibility and interest in vocational rehabilitation (VR) and employment network (EN) services
- Increasing referrals to VR and EN services by 50%
- Demonstrating at least 1% of referred patients receiving services
- Involving facility patient representatives in the establishment and progress of the facility’s plan to address barriers to low referral and underutilization of services
